Working in collaboration with a Supervising Doctor (SD) the Nurse Practitioner (NP) will be responsible for:
Interviewing patients and obtaining historical data.
Performing physical examinations to assess health status and musculoskeletal primary care / sports medicine management problems-- this includes initial patient evaluations, follow-up visits, immediate care clinics, pre-procedure physical examinations, and post-procedure follow-up visits.
Ordering diagnostic studies such as x-rays and imaging, and lab tests such as blood tests, and performing collections of samples such as urines samples and wound cultures.
Writing orders and progress notes.
Diagnosing disease and injury states.
Formulating treatment plans. To be reviewed by the SD
Implementing treatment plans. This includes evaluation and management or problems, routine wound care and suture removal, ordering physical therapy, counseling patients about risks and expected courses of various surgical and therapeutic procedures, referrals, and planning appropriate follow-up care.
The NP may perform the following services only under the direct supervision of the SD:
performing routine minor office based surgical procedures such as repair of simple wounds and lacerations
administration of therapeutic injections
assist in surgery
The NP is authorized to function as my agent per bylaws and/or rules and regulations of the Practice Sites referenced below.
The NP is authorized to write and sign prescription drug orders for the following Schedule: II, III, IV, and V
The NP is authorized to assist me in the performance of, and to perform all further laboratory, diagnostic, and surgical procedures as directed by the SD and as determined by the SD to lie within the scope of the NP ' s license, standard of care, training, skills and experience. The SD may also train the NP to develop additional skills, perform procedures and monitor her performance.
The NP is authorized to provide services to the “Group’s” patients provided the service(s) rendered are within the scope of their license, standard of care and this Delegation of Services Agreement.
The NP may transmit by telephone or written transmittal order to a pharmacist, and orally or in writing on a patient's medical record to a nurse, the SD's order for medications or medical devices requiring a prescription in accordance with her licensure and Family Nurse Practitioner regulations.
California NP License
Strong interpersonal and communication skills.
