I have a project starting in Apple Valley, Ca for in home assessments. The hours and schedules are very flexible and can work around your current schedule. You will be paid per assessment- 45 minutes, we offer mileage reimbursement starting from your home address, and Hotel accommodations if Apple Valley, Ca is a distance for you. You have the flexibility to work 5 days a week, 2-3 days a week, or just the weekends. We have 2000 assessments that need to be completed so there is plenty of work.

***Job Description/Duties*** The Nurse Practitioner /Physician Assistant /MD effectively identifies, evaluates, and address disease prevention and health promotion for Medicare Advantage members while conducting health assessments in the member's primary residence.
The Provider will works in an independent and interdependent relationship with members of the Outcomes Health Information Solutions (OHIS) network of Providers. Collaboration between OHIS internal staff, including recruiting, project management, scheduling, collaborating physicians, and the Clinical Director will be required.
The Provider may also be required, to conduct in-home assessments which focus on the recognition and documentation of acute, chronic, and long-term healthcare issues. Diagnostic Screening tests may be performed based on client need.
The Provider will educate patients and/or families about preventive care, medical diagnosis and use of prescribed medical treatments and/or medications as needed.
Documents and tracks accurate and confidential health risk assessment using paper documentation tool.
***Additional Duties*** Comprehensive Physical Exam to include all body systems, ADL and Fall Risk Evaluation, Depression and Nutritional Health Screening, BMI evaluation and diagnostic laboratory screenings.
***Supervision*** Under the general supervision of a collaborating physician, based on state advance practice regulations. Providers will report to OHIS Clinical Director.
***Knowledge, Skills and Abilities*** * Medicine - Knowledge of the information and techniques needed to diagnose and treat injuries, diseases, and deformities. This includes symptoms, treatment alternatives, drug properties and interactions, and preventive health care measures.
