Vista Post Acute is a 71 bed Skilled Nursing Facility, looking for Therapists to join our team in providing high-quality rehabilitation services at our facility in Hayward, CA. In this role, you will play a vital part in enhancing patients' recovery journeys through compassionate and effective therapy techniques.

Responsibilities 

• Provide direct patient care and support during therapy sessions.
• Monitor patient progress and report changes to the supervising therapist.
• Educate patients and their families on exercises and rehabilitation techniques.
• Maintain accurate documentation of patient treatments and progress.
• Collaborate with healthcare team members to optimize patient outcomes.
• Ensure a safe and supportive environment for all patients.
• Participate in ongoing training and professional development.

Requirements

• State licensure or certification.
• Strong knowledge of anatomy, physiology, and rehabilitation techniques.
• Experience in patient care within a clinical or skilled nursing setting preferred but not required.
• Excellent communication skills and ability to work collaboratively with a team.
• Compassionate approach to patient interactions.
